Dear Valued Patients,

Dr. Jeffrey Harris & Staff appreciate your trust and confidence that you have given to us, and we have been honored to serve your healthcare needs. After twenty two years of faithful service to our community, Dr. Harris has made the decision to officially retire as of October 31, 2023.

With Dr. Harris retiring, patients will need to have their medical records transferred to another provider. Once you have found the physician of your choice, you will need their name, address, phone and fax number. When you have that information, please come by the office to sign a release of medical records. After you have signed the release, we will forward your medical records to the physician that you have chosen.

After October 31, 2023 all medical record requests must be sent to:
